Off label use of direct oral anticoagulants for left ventricular thrombus. Is it appropriate?
A 57 year old gentleman with a history of non-ischemic cardiomyopathy and paroxysmal atrial fibrillation presented with worsening lower extremity edema and dyspnea on exertion. He had been compliant with his medications including rivaroxaban (Xarelto) for atrial fibrillation that he takes with the evening meal daily. His echocardiogram showed an ejection fraction of 10-15% and a new left ventri...

Despite these advantages, clinicians have been concerned over the lack of specific reversal agents for use in the event of a life-threatening bleeding event or when an emergent interventional procedure is required. This concern may limit the use of DOACs, as warfarin can be reversed with the combination of a factor product (fresh frozen plasma or prothrombin complex concentrate) and phytonadion...

New Oral Anticoagulants vs Vitamin K Antagonists: Benefits for Health-Related Quality of Life in Patients with Atrial Fibrillation
INTRODUCTION Acceptance of illness plays a key role, allowing the patient to adapt to the disease and its treatment, and to maintain their health-related quality of life (HRQOL) despite chronic conditions. AIM The aim of the study was to assess the relationship between severity of arrhythmia symptoms, acceptance of illness and HRQOL in patients with atrial fibrillation. METHODS The study in...

Introduction Novel oral anticoagulants (NOACs) represents a group of medications: direct thrombin inhibitor (dabigatran) and direct factor Xa inhibitors (rivaroxaban, apixaban and edoxaban) are increasingly favorized over conventional oral anticoagulants (OAC), vitamin K oral anticoagulants such as warfarin. As requested by some authors the term “NOACs”, should be restricted to “non-vitamin K o...
